Will a counselor ever come right out and say this? Is it ethical to do so?

At this point, my opinion would be to tell the offending party that they have NPD, and to tell the victim that too, and that s/he would do best to consider separation. It might be that the pwNPD, if they have not stormed off, will suggest their own individual therapy…
